If you are currently housed but facing an eviction or in need of rent assistance, do not fill out this form. Instead, apply for rent assistance or get eviction prevention resources on the Pierce County Eviction Prevention Website.

If you have had a Coordinated Entry appointment of any kind in the past 90 days, you do not need another appointment until 90 days after your last appointment.
